Why am I so inconsistent

So recently I was on a huge win streak and climbed straight through plat all the way to diamond. When the game first came out I was also diamond and I took a fairly large break and now i’m back. As soon as I hit diamond I just started losing so many games. Now to the point im in plat 2. Is it just my mental or what can I do to become more consistent?

4 out of the last 5 games I've played someone has left. In one case 3 people left. Riot needs to find a punishment that actually stops people from leaving.

Why can i see my allies loadout and abilites above their head constantly?

Is this a bug or is it meant to be like this from now on? Its veryy distracting espeically if ur whole team is in one place. Is there anyway to get rid of it?
